Reminder:

All officials must wear a BNQ approved throat protector.  It is mandatory as of November 1, 2010.

All OWHA officials, 18 years of age or older, as of September 1, 2011 must submit a satisfactory criminal record check AND criminal declaration form.  The declaration must be submitted with registration.  A satisfactory criminal record check must be submitted no later than November 1, 2011.   If you have already completed a police record or criminal record check for another organization, you may use it.  It must have been completed after April 2011.

The PRC pdf is located on our Forms page.  You can obtain a police record check for the vulnerable sector.  The 'fingerprint check' is not required if the police record check is returned to you noted with "no convictions or charges'.
